New Palliative Care Coordination Pilot | Supporting Residents and Families Every Step of the Way

We know that every stage of life should be lived with dignity, comfort, and connection. That’s why we’re taking part in a new pilot program that enhances our focus on primary, palliative, and end-of-life care, not only for our residents, but also for the families and teams who walk alongside them.

Funded by the WA Primary Health Alliance (WAPHA), this initiative is designed to ensure residents receive timely, high-quality care tailored to their needs. The pilot will run until June 2026 at our Sister Mary Glowrey and St Vincent’s residential aged care homes.

A key part of the program is the appointment of a dedicated Palliative Care Coordinator, a role created to make the care journey smoother, more personal, and better connected.

“This program is about more than improving clinical care. It’s about giving our residents and their families peace of mind, knowing their needs are understood and supported every step of the way. Claire’s role will help us create a more connected, compassionate approach to palliative care, right here in our homes,” said Krupa Shah, Head of Residential Care

Meet Claire Murphy

With extensive experience from her time with the Residential Care Line and a deep passion for palliative and end-of-life care, Claire will be the central point of connection for residents, families, and healthcare professionals.

“Palliative care is about living well, even in the final stages of life. I’m looking forward to working closely with our teams, GPs, and families to make sure every resident receives the care, dignity, and respect they deserve,” said Claire Murphy, Palliative Care Coordinator

Claire’s role includes

  • Bringing together care teams, GPs, and families through regular case conferences.
  • Ensuring communication is clear, compassionate, and consistent.
  • Supporting our staff with training and confidence to deliver the highest standard of care, and
  • Embedding sustainable, person-centred care practices that reflect the Aged Care Quality Standards and our Care with Purpose philosophy.

This pilot program is an opportunity to further enrich the quality of life for our residents, ensuring that at all times, they feel valued, understood, and cared for.
