Restorative Care Pathway (replaces STRC from 1 November 2025)

From 1 November 2025, the Restorative Care Pathway will replace the Short-Term Restorative Care Program under the new Support at Home model.

Sometimes life changes unexpectedly, it could be an illness, a fall, or time in the hospital, and that can make everyday tasks more difficult. The Restorative Care Pathway provides short-term support designed to help older people regain independence and remain living safely at home.

Formerly known as Short-Term Restorative Care (STRC), this pathway is part of the new Support at Home reforms and focuses on restoring function, building confidence and supporting healthy ageing.

Eligible clients can access up to 16 weeks of Restorative Care, alongside their existing Support at Home services. It includes coordinated clinical care – Nursing & Allied Health.

What is Restorative Care?

Restorative Care is a time-limited, goal-focused program that brings together a multidisciplinary team to help you improve daily function and wellbeing. It is ideal for people who:

  1. Have recently returned home from the hospital
  2. Have experienced a decline in strength or mobility
  3. Feel less confident completing daily tasks
  4. Want to prevent further decline and stay independent
  5. Need short-term support after illness or injury

The service is designed to help you maintain or regain independence, not just manage symptoms.

PLEASE NOTE the Restorative Care Pathway does not cover any domestic assistance, social support, gardening and personal care.
Amount funded $14,000 $6,000

Currently, this service is only available in the Perth Metro area – Quinns Rock through to Kwinana and as far as Kalamunda, Armadale and Byford.

How the Restorative Care Program Works

We make the process simple and supportive:

1.   Referral & Assessment

You may enter the pathway through:

  • A My Aged Care assessment
  • A hospital discharge referral
  • A GP or health professional referral
  • A family request supported by your assessment outcome

2.  Goal Setting

We work with you to identify what you want to improve: mobility, balance, everyday tasks, confidence, strength, or independence.

3.  Tailored Support Plan

Your team creates a personalised 12-16 week program (aligned with government guidelines), delivered in your home.

4.  Regular Review

We monitor progress, adjust your program as needed, and celebrate your achievements along the way.

5.  Transition Support

At the end of the program:

  • You may continue with regular Support at Home services, or
  • You may feel confident to continue independently

We ensure a smooth transition whichever path you choose.

Who Is Eligible?

You may be eligible if you:

  • Are aged 65+ (or 50+ for Aboriginal and Torres Strait Islander peoples)
  • Have experienced functional decline
  • May be at risk of entering residential care earlier than needed
  • Want to remain living independently at home
  • Have not recently received similar short-term restorative services

Eligibility is determined through My Aged Care.
