Home Care FAQs

There are many reasons why you might consider companion care: You may be having more trouble doing things that were easy, experiencing a change in your health, or returning home from the hospital.

Everyone values independence, which means you don’t need to give up your freedom just because you might need a hand with the day-to-day.

People want to be safe and happy at home, and Catholic Homes is all about maintaining choice and independence. It includes a comprehensive and ethical case management service.

Eligibility for government-subsidised support at home is based on your needs and circumstances and is determined by an individualised assessment by specialists from My Aged Care and relevant assessment bodies.

Mid West WA | extending to Northampton

Wheatbelt WA | extending from Jurien Bay to Northam, Toodyay, Merredin, Boddington, Narrogin, and surrounds

South West WA | including Bunbury and nearby areas.

If you have more complex needs requiring regular support, a member of the Aged Care Assessment Team will be in contact.

An ACAT assessor will discuss your needs and help develop a support plan. Then, your eligibility for services is determined, and you can choose a quality provider such as Catholic Homes.  

Most elderly care services are subsidised by the Australian Government though you are expected to contribute towards some of the costs.

Costs very depending on type of services and the complexity of care you need; as well as your capacity to pay. 

This may be for you if you are not eligible for a government-subsidised service.

Private Services provides one-on-one support from one of our friendly team members who can help you in the home or accompany you to visit and connect with old friends, enjoy community-based activities, attend appointments, or go shopping.

You can live in the community or in an aged care facility. This service is tailored to suit your individual circumstances.

Through Catholic Homes, you can access aged care services via a Home Care Package/Support at Home. Geraldton clients can also access some services through the Commonwealth Home Support Program (CHSP), which is only available in the Geraldton area for Catholic Homes. As part of your Home Care Package / Support at Home, you can access a wide range of elderly support, including physiotherapy, podiatry, occupational therapy, clinical services, medication management, personal care, domestic assistance, transport services and social support.

To access an affordable home care package/support at home with Catholic Homes, seniors should initially contact My Aged Care.

Home care services do include meal preparation, cooking and clean-up.

We're happy to send one of our client engagement team to visit you, discuss your needs, and also assist with calling My Aged Care with you if required. We will guide you through your home care journey from the initial call, all the way through until you start to receive your services from Catholic Homes.

There are a number of carers with experience in palliative care. We also offer pastoral care in the home when needed.

Follow these 5 easy steps to access Home Care.

Contact My Aged Care

When you contact My Aged Care, they’ll ask you a series of questions to put you on their register. They’ll then arrange for an assessment to determine if you’re eligible for Home Care and what level of Home Care you require. This assessment can be done in your home.

Organise an Aged Care Assessment

To access Home Care packages, you must arrange an assessment with the Aged Care Assessment Team (ACAT), which is part of the Government’s My Aged Care department. Before you contact ACAT, we’re happy to talk with you about the process and what items to have ready, so you’re prepared with all the information you need to register with ACAT.

Receive the outcome of your assessment.

If you meet the criteria, ACAT will notify you of the outcome of your assessment and approve you. You can then contact us, and we’ll send a dedicated Case Manager to your home to visit you. Your Case Manager will go over our services with you and tailor a care plan to match your needs and budget.

Select Catholic Homes as your provider.

After your Case Manager visits you at your home and you’ve decided on a plan together, you can select us as your Home Care provider and sign up for the Home Care package that has been specifically devised for you.

We complete all your paperwork and get your services up and running

Your Catholic Homes Case Manager will activate your referral code so you can start receiving your home support services and funding, which will start straight away.

A Home Care Package provides services tailored to meet your care needs. It helps you enjoy your home for as long as possible as you get older, while giving you choice and flexibility in how your care and services are provided to you at home.

Catholic Homes can offer four types of Home Care packages, from low to high care, and each service is funded at different levels.

Contact one of our experienced case managers, who will help you create a support plan and budget to suit your package fund and needs.

For those who require lower levels of support, their assessment will be done by the Regional Assessment Service. 

A face-to-face or phone assessment will quickly determine if you can assess entry-level services under the Commonwealth Home Support Programme some of which can be provided by Catholic Homes.

The assessor will be able to determine what services you are eligible for once the assessment is complete.

If your needs are straightforward, you might be eligible for entry-level Home Care under the Commonwealth Home Support Programme (CHSP). Your assessor should be able to confirm this during your assessment.

You need a referral to access CHSP services. The RAS assessor will advise provider options and refer to your preferred provider. Catholic Homes may assist you in commencing the services you need.

If you need more complex care, your written confirmation will be given. You’ll receive a letter from the aged care assessment team stating you have been assigned a package, keep this letter and you can contact Catholic Homes if we are your preferred provider. 

Catholic Homes' Short-Term Restorative Care provides a range of care and services to improve your well-being and help you stay independent by improving your ability to complete everyday tasks.

You may be able to receive Short-Term Restorative Care in your home or in the community. The ACAT determines your eligibility.

Depending on your needs, short-term restorative Care can be provided for up to eight weeks (56 days). You may be eligible for Short-Term Restorative Care twice in any twelve-month period.

Yes, you can choose to change your provider at any time. We can make it very easy for you to switch to Catholic Homes.

Catholic Homes offers some in-home nursing care under the Home Care Package/Support at Home programs.

Catholic Homes does not provide 24/7 personal assistance in your own home. However, we have 6 residential aged care homes where we do offer 24/7 high care provision.

Catholic Homes are committed to providing dementia specific care within the home environment when safe to do so.

Mobile physiotherapists are provided as part of our range of extensive Allied Health professionals. The Catholic Homes Allied Health team can be an integral part of your care team, delivering through the Home Care Package/Support at home.
